Enfield CCG is contracting with Enfield Healthcare Cooperative Ltd to invest and develop new, proactive approaches to the management of long term conditions, focusing on preventing future populations from becoming unwell and reducing the demand on acute healthcare services for those who already have one or more LTC.
The contract will initially be for 2 years with an investment of 4 500 000 GBP. There will be an option to extend for 3 years dependent on the outcomes of years 1 and 2. The overall strategy is a 5-year plan.

We aim to:
1) Diagnose our undiagnosed to give them the best possible chance of living with early disease and potentially reversing some aspects of disease;
2) Robustly treat our patients who have one or more LTC to maximise clinical impact and substantially reduce complications and co-morbidities;
3) Significantly enhance our prevention offer across primary, secondary and tertiary modalities to reduce the care and treatment burden.
The contract will initially be for 2 years with an investment of 4 500 000 GBP. The overall strategy is a 5-year plan.

Esboniad:
For 2020/21 and ongoing, the CCG is seeking invest in delivering an improvement of care to patients with long term conditions. The intention is to offer equal access to all Enfield residents, regardless of which practice they are registered with, by offering this service delivery opportunity to a federated model of general practice, where some elements may be delivered by individual practices and others are delivered via other practices or ‘hubs’ on behalf of groups of practices.
The nature of the services to be provided and the need to be provided across primary care within Enfield means that only providers who have access to patient list can deliver this service. Enfield Healthcare Cooperative Ltd provides the federated model that will allow the service to be offered across Enfield.
