Crynodeb

The core purpose of children's centre services is to improve outcomes for young children and their families and reduce inequalities between families in greatest need and their peers (Statutory Guidance and section 2 Childcare Act 2006).

The successful provider will deliver the following:

— Targeted family support through an agreed action plan and package of support to between 100 — 120 families/children at any one time and a minimum of 200 families per year. The level of support provided will be dictated by needs of the families but the following describes the type/level of intervention that the Service should be delivering at any one time:

- 60 % targeted support which includes 1:1 outreach and accessing targeted groups/parenting courses

- 30 % specialist working in partnership with statutory services

- 10 % engagement with those at risk of experiencing poor outcomes

— Targeted family support includes 1:1 outreach into family homes and bespoke targeted groups/services (i.e. evidence based parenting programmes) in children's centres and community venues.

— Will form part of the Connecting Families wider matrix team and support at least 10 families who meet the nationally defined criteria as identified by the local team at any one time, and support these families to achieve the required Connecting Families outcomes.

— Will attend and/or lead Team around the Child (TAC) or family meetings and will allocate a Lead Professional where appropriate.

— Build strong relationships with the Health Visitor, Maternity, Social Care, Adult Services and other Early Years Services including Bright Start Children's Centres to identify those families at risk of experiencing poor outcomes.

— Will ensure representation at all initial child protection conferences, relevant child protection reviews and core group meetings to facilitate joint working and case management as appropriate.

— Strong engagement with families who may be hard to reach.

— Contribute to local Public Health priorities, applying the ‘Making Every Contact Count MECC’ principles and working with Healthy Lifestyle teams to ensure key public health messages are clearly communicated and implemented (see Performance Monitoring Schedule for further details of key Public Health indicators).

— Work with Health colleagues to ensure wellbeing, lifestyle and children's weight management services reach families that need them the most by ensuring families are identified, signposted and referred through established pathways and that services are delivered flexibly and in partnership, with shared outcome measurements.

— Will intervene opportunistically offering locally agreed screening tools for brief interventions and signpost families onto appropriate local services to support stop smoking, alcohol and substance misuses services, mental health, sexual health services and weight management services

— Will participate in and support 2 year checks / reviews with Health Visitors and Early Years teams where the Service is working with the family at the same time.

— Will support parents to access good quality early years provision and take up their 2, 3 and 4 year free entitlement to early education. With support from the Local Authority Business Support team, they will track and work with those disadvantaged families not accessing this.

— Will support parents/carers to volunteer, develop peer support networks and build community capacity.

— Will support parents/carers to enable them to access financial advice, budgeting support, debt advice and maximise income.

— Will support parents/carers to take up training and employment opportunities and to achieve financial independence and work with employment partners who provide community learning courses and programmes.

— Will deliver services from a community hub co-locating with Health Visitors where possible to enable access to a

A Bath West Children's Centre service is required to work in close partnership with the Council managed Children's Centre service to deliver the following outcomes for expectant parents and families with children up to 5 where their needs cannot be met through universal provision:

— Parents increase their capacity to parent well.

— Children are physically and emotionally healthy.

— Parents/carers are able to keep their children safe.

— Parents have a good level of personal, education and employability skills.
