Brighton and Hove CCG wish to commission an integrated model of care (for adults and children) which will encompass the following:
- Triage by appropriate clinicians;
- Services currently provided under the Minor Surgery DES (national specification) and Community Dermatology LES (the local specification currently includes - benign naevi moles, bowens disease, dermofibroma /histocytoms, epidermoid/ pillar cysts, lipomata,pyogenic granuloma, keratosis, skin tags, skin lesions, viral cutaneous horns, diagnosed and low risk basal cell carcinomas);
- All dermatology adult and children, elective out-patients (including - routine, urgent, 2WW) delivered along integrated pathways between community and hospital settings as appropriate;
The need for change is driven by a number of national and local factors. National needs assessment work indicates an on-going increase in the Dermatology workload and the need to improve self-care and the role of primary care. We need to develop a model that manages increasing activity whilst delivering high standards of care and patient satisfaction within an affordable budget.
The current Integrated Dermatology services have been running on a pilot basis since July 2010. They are provided under a contract with Brighton Integrated Care Services (BICS - as prime contractor) who then subcontract to Brighton and Sussex Hospitals Trust (BSUH) for consultant input. The current services include the management of Minor Surgery DES04 part A, the Dermatology LES, Community services run by GPSWIs (Brighton and Hove Skin Care) and all elective outpatient activity. It also includes patient Triage. Going forward, it is proposed that the new service should retain all of these components but also include Paediatric Dermatology out patients.
Current Key Performance Indicators highlight issues with the length of wait for the community clinics and whilst this was set at a very ambitious 3 week target there are vulnerabilities around the GPwSI workforce that could be addressed within a more integrated model of working.
The key objectives of the procurement are:
- To further develop an integrated service so that patients are treated in the right environment with the right skills and as one stop, where possible;
- To improve support to patients to self manage their conditions ;
- To strengthen the skills and knowledge in primary care to support patients with skin conditions effectively;
- To deliver affordable and Value for Money (VfM) services;
- To develop high quality effective care for patients that improves their quality of life;
- To provide faster access to diagnosis and treatment and to reduce waiting times;
- To benefit from a more robust contracting basis;
- To develop innovative approaches to service delivery;
- To contribute to equality, sustainability and social value benefits.
The service does not currently include all of paediatric out-patient activity and there are pathway efficiencies to be gained by including this.
The current budget for the service has no incentives to improve quality, develop innovative solutions or manage activity and costs more appropriately.
The estimated contract values stated are for the total of the 3 year contract term.
