First Community Health and Care (FCHC) is participating in the development of an integrated care pathway (ICP) involving Surrey and Sussex Healthcare NHS Trust, Sussex Community Foundation Trust, Surrey and Borders Partnership, South East Coast Ambulance Service and Crawley CC and Horsham and Mid-Sussex CCG.

A critical early work stream in the development of this ICP being established in a financially challenged system has been a rapid piece of work to develop a financial recovery plan and a robust governance structure to support the delivery of this plan, with initial operating plan submissions from the CCGs showing significant gaps to control totals. FCHC have been a constructive partner in the early development of this plan.

Concurrently, East Surrey CCG is in the process of changing STP alignment towards Surrey Heartlands from Sussex and East Surrey this adds further complexities to the system change programme.

The combination of the changing commissioner landscape and development of new ICP partnership arrangements have led East Surrey CCG to conclude that a competitive process would not be conducive to the effective or efficient delivery of the services at this time. Most notably, financial recovery and sustainability is considered to be reliant on accelerated development of the future system and service configurations. There is a lack of certainty of the defined scope of provision for community partners, at this point of service and pathway redesign, that can be more readily managed and achieved with existing system partners.
