Medical consumables. Framework for the Provision of Consumables for Interventional Neuro Radiology and associated accessories to members of the Shelford Group to include:- Detachable Coils; Carotid Stents; Guide Catheters, standard, distal access, intracranial access; Micro Catheter; Flow Guided Micro Catheter; Micro Guide Wire- neuro use only; Flow Diverters; Aneurysm Devices; Thrombectomy/Clot Retrieval Devices; Balloon Catheters intracranial; Embolization Material for intracranial use; Rapid Exchange Angioplasty Balloons for carotid use. The Framework Agreement shall be 24 months duration with possible 2 x 12 month extension options i.e. a maximum period of 4 years duration. Should The Shelford Group choose to extend the Framework Agreement the existing terms and conditions of contract will apply for the extension period(s).

(a) Appropriate statements from banks or, where appropriate, evidence of relevant professional risk indemnity insurance.
(b) The presentation of balance-sheets or extracts from the balance-sheets, where publication of the balance-sheet is required under the law of the country in which the economic operator is established.
(c) A statement of the undertaking's overall turnover and, where appropriate, of turnover in the area covered by the contract for a maximum of the last three financial years available, depending on the date on which the undertaking was set up or the economic operator started trading, as far as the information on these turnovers is available.

Capasiti technegol
(a) A list of the works carried out over the past 5 years, accompanied by certificates of satisfactory execution for the most important works. These certificates shall indicate the value, date and site of the works and shall specify whether they were carried out according to the rules of the trade and properly completed. Where appropriate, the competent authority shall submit these certificates to the contracting authority direct.
(b) A list of the principal deliveries effected or the main services provided in the past three years, with the sums, dates and recipients, whether public or private, involved. Evidence of delivery and services provided shall be given: — where the recipient was a contracting authority, in the form of certificates issued or countersigned by the competent authority, — where the recipient was a private purchaser, by the purchaser's certification or, failing this, simply by a declaration by the economic operator.
(c) An indication of the technicians or technical bodies involved, whether or not belonging directly to the economic operator's undertaking, especially those responsible for quality control and, in the case of public works contracts, those upon whom the contractor can call in order to carry out the work.
(d) A description of the technical facilities and measures used by the supplier or service provider for ensuring quality and the undertaking's study and research facilities.
(e) Where the products or services to be supplied are complex or, exceptionally, are required for a special purpose, a check carried out by the contracting authorities or on their behalf by a competent official body of the country in which the supplier or service provider is established, subject to that body's agreement, on the production capacities of the supplier or the technical capacity of the service provider and, if necessary, on the means of study and research which are available to it and the quality control measures it will operate.
(f) The educational and professional qualifications of the service provider or contractor and/or those of the undertaking's managerial staff and, in particular, those of the person or persons responsible for providing the services or managing the work.
(g) For public works contracts and public services contracts, and only in appropriate cases, an indication of the environmental management measures that the economic operator will be able to apply when performing the contract.
(h) A statement of the average annual manpower of the service provider or contractor and the number of managerial staff for the last 3 years.
(i) A statement of the tools, plant or technical equipment available to the service provider or contractor for carrying out the contract.
(j) An indication of the proportion of the contract which the services provider intends possibly to subcontract.
(k) With regard to the products to be supplied: (i) samples, descriptions and/or photographs, the authenticity of which must be certified if the contracting authority so requests; (ii) certificates drawn up by official quality control institutes or agencies of recognised competence attesting the conformity of products clearly identified by references to specifications or standards.

Expressions of Interest must be submitted through the e-Tendering portal at Delta e-Sourcing. KCH or any of the member trusts must not be contacted directly in relation to this contract except through the identified channels as at Section I.1.
When required, pricing must be in Sterling and payments under the contract will be made in Sterling. The contract is expected to last for 2 years with extension options up to 24 months. The initial tendering price must be held for a minimum of two years Trusts will award business on a two-year or year-by-year basis and reserve the right to change suppliers at the anniversary of their award. The contract will be governed by English law. KCH reserves the right to annul the tendering process and not award a contract.
If and when this requirement is offered to tender, this may be done in whole or in part via electronic means using the Internet. The contract is for the benefit of other participating NHS bodies (whether acting individually, or on behalf of, or together as members of any consortia) who are members of the Shelford Group as may change from time to time.
The selected suppliers will be expected to supply all Consumables, Products and Services from their range as are required by the contracting authority from time to time. The entire ranges of their products and Services will be required if submitting an offer for any/all Lots. Future additions to their ranges will also be included.
For means of clarity this is a Shelford Group framework agreement.
The tender exercise will be co-ordinated through a procurement panel of representatives from member Trusts. Further information will be sent out in the tender documentation.
Bidder clarifications, if applicable, will be held after receipt of the Invitation to Tender Documents have been returned and suppliers must make themselves available for them.
The KCH does not bind itself to accept the lowest offer or any offer either in part, each item for this purpose being treated as separately.
The KCH cannot be held responsible for any costs incurred by offerors in relation to the preparation of their and any subsequent post offer clarifications.
Suppliers must use the file reference F/570/15 in all correspondence. Suppliers must follow the instructions fully. Suppliers must complete their PQQ document set before the deadline as set out in this notice.
Suppliers must comply with all of the requests within this OJEU notice and subsequent tender requirements or they may not be considered.
