Older Adults community teams in Adults' Health and Care routinely work with individuals aged 65+, and their carers, who are often facing difficult and complex circumstances. The Community Wellbeing Worker service will provide short term, preventative support to these individuals, and will draw upon local community resources as required.

The primary aim of the service is to enable individuals to increase their independence and to reduce their potential need for current or future involvement from adult social care services.

The Service has been divided into 8 lotted contracts, with geographical breakdowns:

Lot A - Andover

Lot B - Basingstoke

Lot C - Eastleigh

Lot D - Fareham & Gosport

Lot E -Havant North & South

Lot F - New Forest East & West

Lot G - Rushmoor

The Council have received funding up to the value of £240,000.00 for a period of one year. The funding will be divided between the Lots at a value of £30,000.00 per Lot.

There will be an option to extend the service by an additional year (12 months) up to the value of £240,000.00 should additional funding be agreed however there is no guarantee that the additional funding will be given. If this funding is agreed, the total contract value of the service is £480,000.00 (£60,000.00 per Lot, per annum).
