Disgrifiad caffael
Cartref gofal preswyl 32 ystafell wely yw Dolwen a adeiladwyd yn y 1960au. Mae’n ei chael yn anodd diwallu’r anghenion cymhleth sydd gan ddinasyddion sydd angen gofal preswyl. Pwrpas y prosiect yw darparu eiddo addas i’r diben sy’n fodern a hygyrch ac sydd a digon o le ar gyfer darpariaeth bresennol a darpariaeth yn y dyfodol. Bydd yn defnyddio technoleg ddigidol a Deallusrwydd Artiffisial. Bydd mewn lleoliad da i ddarparu gofal yn nes at adref a diwallu anghenion iechyd a gofal pobl leol gan gynnwys anghenion heb eu diwallu, h.y. darpariaeth i henoed bregus eu meddwl, a chefnogi darpariaeth gwasanaeth yn yr adeilad ei hun ac yn y gymuned leol.
Ar hyn o bryd, mae angen rhoi sylw i’r materion canlynol yng nghartref Dolwen
- Ystafelloedd bach sy’n cyfyngu ar allu’r darparwr gwasanaeth i ddiwallu anghenion y preswylwyr, er enghraifft, mae ystafelloedd sy’n addas ar gyfer teclynnau codi, ond nid yw hyn yn wir trwy’r cyfleuster i gyd. Mae’r cyfyngiad hwn yn cyfyngu ar bwy ellir darparu llety iddynt, yn dibynnu ar eu hanghenion.
- Nid oes gan yr un ystafell en-suite, ac nid ydynt yn cynnig gofod byw digonol, na’r cyfleusterau a ddisgwylir gan safonau modern.
- Mae’r darparwr gwasanaeth yn ei chael hi’n anodd diwallu anghenion cymhleth, sy’n golygu nad oes modd darparu llety i gleifion fel yr hoffant.
Mae amrywiaeth eang o ffactorau yn dylanwadu ar y cynnig: - cynnydd disgwyliedig ym mhoblogaeth hŷn Sir Ddinbych i 30.9% erbyn 2040; gan gynnwys na fydd y cyfleusterau presennol yn addas ar gyfer anghenion y preswylwyr; gwella darpariaeth gwasanaeth; dod â gwasanaethau’n nes at y gymuned; gwella’r ddarpariaeth bresennol ar gyfer pobl hŷn a’r angen
i oresgyn cyfyngiadau fel mynediad, lle a pharcio, a bodloni her a nod Sir Ddinbych i fod yn garbon niwtral erbyn 2030.
Mae’r prosiect hwn, a gaiff ei arwain gan Gyngor Sir Ddinbych (CSDd) yn cydweithio â Thai Cymunedol, Cyngor Gwasanaethau Gwirfoddol Sir Ddinbych a Bwrdd Iechyd Prifysgol Betsi Cadwaladr ar brosiect i adolygu datblygiad posibl Cartrefi Gofal Preswyl Dolwen. Mae’r prosiect yn cynnig estyniad a gwaith moderneiddio i gartref Dolwen gan gynnwys
- Capasiti preswyl i henoed bregus eu meddwl, h.y. datblygu adain dementia
- Darparu model mân ofal ychwanegol yn y cyfleuster tai pobl hŷn sy’n ffinio â Dolwen, mewn partneriaeth â Thai Cymunedol.
- Model prif ganolfan a lloerennau o gefnogaeth gymunedol ar sail allgymorth a mewngymorth ar y cyd â Thai Cymunedol a Chyngor Gwasanaethau Gwirfoddol Sir Ddinbych.
- Uwchraddio a moderneiddio’r cyfleuster preswyl presennol o 32 i 50 gwely. Bydd hyn yn cynnwys estyniad a bydd yn galluogi’r llety presennol i gael ei ailddatblygu i sicrhau ei fod yn diwallu anghenion cymhlethdod yn y dyfodol, e.e. teclyn codi ym mhob ystafell a chyfleusterau en suite.
- Gwella darpariaethau presennol cymorth dementia yng nghymuned Dinbych a’r ardal gyfagos
Dolwen is a 32 bedded residential care home that was build in the 1960s which is struggling to meet the complexity of need that is being experienced by citizens requiring residential care. The purpose of the project is to deliver fit for purpose premises that are modern, accessible, have sufficient space for current and future provision, utilise digital and AI technology, are well located to deliver care closer to home, meet the health and care needs of local people including unmet need I.E residential EMI provision and supports service delivery both building based and within the local community
Currently Dolwen has the following issues that need to be addressed
- Small rooms which limit the service providers to meet the needs of the residents, for example there are rooms which can accommodate hoists however not throughout the facility. This restriction limits who can be accommodated depending on their needs.
- None of the rooms are en-suite and do not offer the size of living space or facilities which would be expected in modern standards.
- The service provider struggles to meet the complexity of the needs meaning patients cannot be accommodated in the way which they would like.
The proposal is influenced by a broad range of factors: - predicted increase in the older population of Denbighshire to 30.9% by 2040; including the current facilities being unsuitable for the needs of the residents; improving service delivery; bringing services closer to the community; enhancing the current provision for older people and the need to overcome limitations such as access, space and parking and meet Denbighshire's challenge and objective to be carbon neutral by 2030.
This project which is led by Denbighshire County Council (DCC) is working collaboratively with Community Housing, Denbighshire Voluntary Services Council (DVSC) and BCU on a project to review the potential development of Dolwen Residential Care Home. The project proposes both an extension and the modernisation of Dolwen to include
- Residential EMI capacity I.E developing a dementia wing,
- Provide an extra care lite model to the older peoples housing facility bordering Dolwen in partnership with Community Housing.
- Hub and Spoke model of community support on an outreach and in reach basis in collaboration with Community Housing and DVSC.
- Upgrade and modernisation to the current residential facility from 32 to 50 beds. This will include an extension and will enable the current accommodation to be redeveloped to ensure that the accommodation meets future needs of complexity e.g. hoist in every room and ensuite facilities.
- Enhance the current provisions of dementia support within the community of Denbigh and t's surrounding area.
