II.2.4) Disgrifiad o’r caffaeliad
The purpose of this tender is to commission housing related support within Ty Parc, a 24 hour homelessness hostel accommodating single people, couples and families with children. In terms of this contract specification the term families encompasses both couples and families with children. Support is provided to individuals/families, who have found themselves homeless, to find suitable accommodation as well as maximising levels of independence enabling people to develop the skills to successfully live independently within their own home.

The contract will run for a period of four years, commencing 1st April 2019 with an option to extend for a further 36 months at 12 month intervals. The contract will therefore be renewed between 1st April 2023 and 1st April 2026 (dependent on whether the extension periods are utilised).
